The JVS LIFE (Learning
Innovations for Enrichment) program is a person-driven
approach to ensure that individuals with disabilities
have a voice regarding their future. The program promotes
self-determination and the right of people with special
needs to choose their own destiny and who will provide
services to meet their dreams and desires.
JVS has provided supports coordination
services to area residents with developmental
disabilities, mental illness and/or multiple disabilities
since
1979.
Realizing
life's potential
With the assistance
of JVS Supports Coordinators, people with disabilities
design plans to explore their horizons and pursue their
dreams. In partnership with each individual,
JVS does the following:
Conducts an initial,
pre-planning discussion
Holds a Person-Centered
Planning Meeting and develops a supports plan
Ensures that an
individual's choices and decisions are made without
compromising health and safety issues
Provides referrals
to requested supports such as medical, social, vocational,
educational and residential services, legal representation,
transportation and respite care.
Assists in developing
natural supports such as a circle of family, friends
and other interested parties
Monitors the implementation
of the supports plan and its effectiveness
Provides advocacy
services and support to individuals and their family
members or guardian
