ANNOUNCEMENT OF STAFF OPENING

FOR

CASE COORDINATOR

JVS announces a staff opening for a full-time (40 hours per week) Case Coordinator in its Rehabilitation Services department located in Detroit.

QUALIFICATIONS:

Bachelor’s degree in psychology, social work, recreation, or related field required. One year related work experience in case coordination, training and assessment with individuals having developmental, emotional, and/or physical impairments required. Experience with the U.S. Department of Veterans Affairs in counseling or advocacy with U.S. military service veterans preferred. Veterans preferred. Working knowledge of Microsoft Word and Excel required. Interpersonal and presentation skills sufficient to provide training, communicate effectively with participants, funding sources, and staff. Composition skills must be sufficient to prepare required documentation. Math skills sufficient to score and interpret testing instruments. Driving record must meet safe driving standards as established by Agency insurance carrier. Valid Michigan driver’s license.

ESSENTIAL DUTIES:

1.

Review participants’ progress toward achieving their individual assignments and goals on a regular basis and coordinate timely communication with family, referring counselors and/or any other relevant individuals.

2.

Complete intake and orientation sessions as assigned.

3.

Provide instruction to participants according to defined curriculum to enhance life and/or work skills as defined by program master schedule and participate in program outings and events with participants as scheduled.

4.

Develop a menu of recreational, leisure, volunteer and community supports resources.

5.

Record observational and behavioral data and compile into service notes and/or reports.

6.

Under the guidance of a Master Degreed Counselor/Psychologist or equivalent develop, coordinate, monitor, and revise as needed, individual plans of service, including specific intervention programs as necessary to modify and teach more functional behaviors.

7.

Under the direction of the Supervisor, Psychological Services, administer, score, interpret and report results of selected psychometrics and other assessment tools as defined by curriculum.

8.

Participate in outcome focused, cooperative group brain storming, consultation and problem solving meetings to enhance the participants’ program outcomes.

9.

Work with the Supervisor, Rehabilitation Services to market program and related basic assessment services.
 
10. Establish and maintain positive and outcome-oriented relationships with participants and their involved others, other team members and funding sources.
 
11. Serve on Agency committee(s) as appointed.
 
12. Transport consumers as needed.

WORKING CONDITIONS:

1.

Work involves the ability to work flexible hours in community and facility settings as well as traveling to local sites using own vehicle.

SALARY AND RELATED:

Salary is based on experience. Benefits include a Blue Cross/Blue Shield PPO or HMO for individual coverage. Family coverage is available under a premium-sharing plan. Option is available to substitute $1,560 per year in lieu of health insurance. Other benefits include vacation, sick leave, life insurance, long-term disability, and retirement when eligibility requirements are met.
